Командиром тральщика Т-216 был старший лейтенант Д.Г. Степанов. Входил во 2-й дивизион БТЩ главной базы флота (Таллин). Тральщик погиб 6 июля 1941 года, подорвавшись на минах, при обеспечении вывода подводных лодок. Часть экипажа Т-216 осталась жива.
